Application for Leave of Absence: 

Please use this to apply for permission to take your child out of school during term time. All Leave of Absence applications MUST have an accompanying letter to explain the reason for the request.  Holidays are not authorised during term time.

Free School Meals & Pupil Premium - Eligibility and Application Form: 

This enables the school to check for Free School Meal Eligibility and also if additional money (Pupil Premium) can be claimed from the Government. Further information about how the school spends these monies can be found on the Pupil Premium page of our website.  

Special Dietary Needs:  

This must be completed and returned to the school office if your child has any special dietary needs.

Medication: 

If your child requires prescribed medicine during the school day, this must be completed and handed in to the school office with the medication.  We are unable to administer medication if this form has not been completed.

Permission to walk home: 

If your child is over the age of 10, you can complete this form to give permission for them to walk home without an adult.  

DBS Application form:

If you would like to volunteer at our school and you will be working with the children unsupervised, you will need to complete a DBS.  Please complete this form to start the application process.

DBS Update Service form:

If you would like to volunteer at our school and you will be working with the children unsupervised, you will need to have a DBS.  If you are already on the DBS update service.  Please complete this form to allow us to check your status and update our records accordingly.

Volunteer form:

If you would like to volunteer at our school, we would like to know how you can offer your time to enhance the learning of our pupils.  You can volunteer to read, help in the library, help on visits etc.
